Only in some rare circumstance where it is vitally critical do we offer to show what has been written down. At the end of the game, then the worksheets are available for both players. Our house rule is, if at the end of the game it is discovered that you placed a HIP unit in the wrong hex and that is critical to the game, as in your example, then you forfit the game and your win becomes a loss. Of course if your opponent still wins dispite your critical error, then it's moot. Because of this we are very careful to make sure what we write down as HIP and we are equally careful to place that HIP where we planned. I can't tell you how many times I've been forced to place a HIP unit on the map in a hex that I wrote down by mistake during setup. If we write it down before play begins, then that is where it goes once play has started, error or not.

No. We usually don't ask but if someone wants to verify something, then sure, I'll show him.

If you can't keep the dice on the playing area, then you are not paying attention. We solved that problem by using a dice glass to roll in. Solves the problem. If on the rare occasion one of the dice is cocked and does not land flat in the glass, then you roll again. Only rolls where both dice land flat in the glass count. Not only that but if you leave the dice in the glass after rolling then you always know where the dice are.

Dice rolls are always a problem, I'm thinking about going with a dice tower. Of all the methods this to me seems to be the best for fair die rolls and keeping the die on the table.

If one player is forced to quit a game prior to the scenario's scheduled end due to no fault of their own, would you consider this a forfeit or a "draw?"

There is usually a record of the game, so you wait for a certain amount of the for the player who dropped out to re-establish contact and resume the game. If he refuses to do so then you declaire the other player the winner by forfeit. There is also the option of declairing the interuppted game a "non-game" and have them begin the scenario again. There are some players who if they are losing will "accidently lose contact", you can't reward them with a draw.

But if a person has to quit "due to no fault of their own" and is unable to resume the game at a later time then I would opt for a draw. However, I do play with people who would insist, that if you can't finish a game for any reason, then it is a loss. I'm a little easier than that.
